Supporting Your Independence with CFSS and 245D Services
We offer two key services: Community First Services and Supports (CFSS) and 245D
licensed supports, both aimed at helping individuals live better, safer, and more independently.
CFSS (Community First Services and Supports)
This service replaces PCA and gives you more control over your care. You can choose your caregiver, including family or a friend, and decide how services are delivered.
What CFSS Helps With
Activities of daily living (bathing, dressing, mobility)
Health-related tasks under a nurse’s direction
Light housekeeping
Meal prep and planning
Help with transfers and positioning
Shopping and community participation
We’ll assist you with the full CFSS process—assessment, service delivery plan, and ongoing support.
245D Licensed Services
Our 245D services support people with disabilities and seniors through structured and professional care delivered at home or in the community.
